Although he did not achieve the success he achieved with the club with his country and was usually fitter with Al Ahly than Egypt, he was always included in the starting lineup. El-Khatib played for Egypt a total of 54 caps and scored 24 goals. He played with his country in the Africa Cup of Nations in the 1974, 1976, 1980, 1984 editions, and he was a part of the team that won the 1986 Africa Cup of Nations title. He also played in the 1984 Olympics in Los Angeles.[3]

After retirement, he worked in sport management where he was elected Al Ahly's board member for more than a period. In 2004, he became the vice-president of Al Ahly, before being elected as president in 2017.[4]
