Frančesko I Medičio ir Joanos, Austrijos hercogienės dukra. Buvo viena iš septynių vaikų, tačiau tik Marija ir jos sesuo Eleonora Mediči sulaukė pilnametystės.
1600 metų spalį ištekėjo už Prancūzijos karaliaus Henriko IV, kai jo santuoka su Margarita de Valua buvo pripažinta negaliojančia, ir tapo antrąja jo žmona.
Vedybos nebuvo labai pasisekusios, konfliktavo su Henriko meilužėmis, o ypač su Katerina Henrieta de Balzac d'Entragė, kurią Henrikas kažkada buvo prižadėjęs vesti.
